Student Update Form

Students who cannot make enrollment changes on MyUCLA may submit a Student Update Form to with the following:

  • Your UCLA ID number
  • The Class ID
  • Lecture name/number
  • Session (ex. A6)
  • Any Discussions or Labs with ID number

Drop Request Form

Students requesting to drop a non-impacted Summer Course in the last 2 weeks of the session must email with one of the following:

  • The instructor’s authorized signature
  • A forwarded email from the instructor approving of the drop
  • OR the syllabus stating the date of the final has not yet occurred

Confidential Financial Statement for I-20 Requests

International students who are subject to I-20 requirements must complete, sign, and submit this form during the I-20 application process to verify evidence of financial support in order to be issued a UCLA I-20.

F-1 SEVIS Transfer-In Form

International students 1) who are currently on an F-1 visa attending an institution in the U.S. and 2) whose I-20 ends prior to the start of their participation in UCLA Summer Sessions must complete the F-1 SEVIS Transfer-In Form.
