Student Update Form
Students who cannot make enrollment changes on MyUCLA (due to holds, enrollment blocks, technical issues, leave of absences, etc.) may submit a Student Update Form to email@example.com. Make sure to include your UCLA ID number, class ID, lecture name/number, session, and any applicable Discussions or Labs with ID number.
Drop Request Form
Students who want to drop a non-impacted Summer Course in the last two weeks of the session must submit this form to firstname.lastname@example.org including the instructors verified signature, a copy of the syllabus clear stating the date of the final, OR a forwarded email from the instructor.
Confidential Financial Statement for I-20 Requests
International students who are subject to I-20 requirements must complete, sign, and submit this form during the I-20 application process to verify evidence of financial support in order to be issued a UCLA I-20.
F-1 SEVIS Transfer-In Form
International students 1) who are currently on an F-1 visa attending an institution in the U.S. and 2) whose I-20 ends prior to the start of their participation in UCLA Summer Sessions must complete the F-1 SEVIS Transfer-In Form.
