Student Update Form
Students who cannot make enrollment changes on MyUCLA may submit a Student Update Form to firstname.lastname@example.org with the following:
- Your UCLA ID number
- The Class ID
- Lecture name/number
- Session (ex. A6)
- Any Discussions or Labs with ID number
Drop Request Form
Students requesting to drop a non-impacted Summer Course in the last 2 weeks of the session must email email@example.com with one of the following:
- The instructor’s authorized signature
- A forwarded email from the instructor approving of the drop
- OR the syllabus stating the date of the final has not yet occurred
Confidential Financial Statement for I-20 Requests
International students who are subject to I-20 requirements must complete, sign, and submit this form during the I-20 application process to verify evidence of financial support in order to be issued a UCLA I-20.
F-1 SEVIS Transfer-In Form
International students 1) who are currently on an F-1 visa attending an institution in the U.S. and 2) whose I-20 ends prior to the start of their participation in UCLA Summer Sessions must complete the F-1 SEVIS Transfer-In Form.
