Downloadable Forms

Students who cannot make enrollment changes on MyUCLA may submit a Student Update Form to info@summer.ucla.edu with the following:

• Your UCLA ID Number (9-digits)
• Class ID (9-digits on Schedule of Classes course page)
• Lecture Name and Number (e.g. PSYCH 10)
• Session (e.g. A6)
• Any Discussions or Labs with ID Number

Students may drop impacted courses until Friday of week 1 (5pm PDT).

Students dropping NON-impacted courses in the final two weeks must email info@summer.ucla.edu with one of the following:

• The instructor’s verified signature
• A forwarded email from the instructor approving of the drop
• OR the syllabus stating the date of the final has not yet occurred

*Note: students are financially liable if dropping past the Refund Deadline

International students who are subject to I-20 requirements must complete, sign, and submit this form during the I-20 application process to verify evidence of financial support in order to be issued a UCLA I-20.

International students 1) who are currently on an F-1 visa attending an institution in the U.S. and 2) whose I-20 ends prior to the start of their participation in UCLA Summer Sessions must complete the F-1 SEVIS Transfer-In Form.
